Our teams consistently exceed national benchmarks in childhood immunizations, hypertension control, tobacco screening and cessation, healthy weight screening and counseling, depression screening and follow-up, and early entry into prenatal care with healthy birth outcomes. In addition to the administrative offices in Glenwood Springs, there are eight clinical locations spread across the area including hub clinics in Avon, Basalt, Glenwood Springs, Gypsum, and Rifle. There are three School Based Health Centers located throughout the service area as well.
